چکیده

مقدمه: یکی از رسالت‌های دانشگاه‌های علوم پزشکی کشور توانمندسازی دانشجویان است. که دستیابی به این امر مستلزم تحول در نظام آموزشی کشور می‌باشد. در این مسیر چالش‌هایی نیز وجود دارد که نیازمند به کار گیری تدابیری به منظور شناسایی و رفع آن‌ها است. بنابراین مقاله حاضر با هدف توصیف و توضیح برخی از چالش‌های آموزش بالینی در علوم پزشکی تبیین شده است. روش کار: مطالعه حاضر یک مطالعه مروری اولیه است که با مطالعه حدود سی و هشت مقاله انگلیسی و فارسی مرتبط با موضوع آموزش بالینی بین سال‌های 1990 تا 2013 به کمک موتورهای جستجوی علمی مختلف از جمله PUBMED-PREQUEST-ELSEVIER-SID-IRANMEDEX تدوین شده است. در این مطالعه فقط مقالاتی مورد بررسی قرار گرفته که به تشریح چالش‌ها و مشکلات آموزش بالینی پرداخته‌اند. مقالاتی که محتوای تکراری داشته و شرایط ورود به مطالعه را نداشتند (320 مورد) از مطالعه حذف شدند. کلمات کلیدی برای جستجو بر اساس نظام مرجع نویسی بین‌المللی کتابخانه های علوم پزشکی (NLM) عبارت بودند از: clinical education- clinical training-challenges & clinical education-method & clinical education. یافته ها: چالش‌های آموزش بالینی پرستاری را می‌توان به چالش‌های خرد (در سطح دانشگاه) و کلان (در سطح ملی) یا به چالش‌های مربوط به مربی، فراگیر، برنامه آموزشی و محیط بالینی تقسیم کرد که در متن مثال‌هایی از آن‌ها آورده شده است و الگوی تأثیر گذاری آن‌ها نیز به صورت یک مدل علی ترسیم شده است. نتیجه گیری: آموزش بالینی پس از این که توسط چالش‌های خرد و کلان ایجاد شده در محیط بالینی تحت تأثیر قرار گرفت به شرایط موجود تبدیل می‌شود که می‌تواند خوب، متوسط یا ضعیف باشد، اما هیچ‌گاه در حد عالی نخواهد بود.

An obligation of medical sciences universities is to empower students. Relevant changes in country’s educational system are required to attain this goal. Obstacles on the way call for strategies to identify challenges and overcome them. The present article, thus, aims to describe and also explain a number of challenges posed in medical clinical training.
Materials and Methods:
This is a preliminary review article which has surveyed 38 related English and Persians articles published on clinical training in 1990-2003. The articles were retrieved by ProQuest, PubMed, Iranmedex, SID, and Elsevier databases. Only articles that dealt with the problems and challenges of clinical training were studied. Articles containing redundant content (320 cases) were excluded from the study.
Results:
The present paper has discussed the importance of effective clinical training, and has explained and categorized the challenges posed to it. Challenges of nursing clinical training can be divided into micro challenges (at university level) and macro challenges (at national level) or into the challenges related to trainer, trainee, training program, or clinical setting. Examples of these challenges are presented in the text and their affectivity pattern has been drawn ini a causal model.
Conclusion:
Clinical training becomes part of the present condition after meeting micro and macro challenges posed in clinical settings. This condition may be good, moderate, or weak but never perfect.
