چکیده
مقدمه: پرستاران، به عنوان مراقب اصلی افراد مبتلا به کووید19، در معرض چالش‌ها و تعارض­های اخلاقی مراقبت از بیماران خود قرار گرفتند؛ چنانچه هم­زمان با مراقبت از بیماران قلبی - عروقی مبتلا به کووید19، باعث می­شد که بار چند برابری از نظر مراقبتی و اخلاقی، بر پرستاران این بیماران، وارد گردد که موجب فرسودگی و تحلیل روانی این افراد، می­گشت؛ بنابراین مطالعۀ کیفی حاضر، با هدف  درک چالش ­های اخلاقی پرستاران مراقبت کننده از بیماران قلبی مبتلا به کووید 19، انجام شد.
روش کار: پژوهش حاضر، یک مطالعۀ کیفی بود که با استفاده از تحلیل محتوای مرسوم، انجام شد. شرکت­کنندگان، شامل پرستاران شاغل در بخش مراقبت ویژه و ایزولۀ تنفسی بیماران مبتلا به کووید، در بیمارستان تخصصی قلب شهید دکتر چمران شهر اصفهان، بودند. نمونه­ گیری به صورت هدفمند و از فروردین تا خرداد 1401، انجام شد. مصاحبه ­های نیمه­ساختاریافته و عمیق، با همکاری شرکت­کنندگان، صورت پذیرفت. تجزیه و تحلیل داده­ ها، با استفاده از روش تحلیل محتوای کیفی گرنهایم و لاندمن و با استفاده از نرم افزار مکس کیو دی ای نسخۀ 0/4/200، انجام شد.
یافته ­ها: مشارکت ­کنندگان در این پژوهش، شامل10 پرستار شاغل در بخش مراقبت­ های ویژه، بودند. در مجموع، تعداد 204 کد اولیه، به دست آمد که در 8 زیرطبقه و 3 طبقۀ اصلی، شامل: «مراقبت قانونی یا مراقبت وجدانی»، «خودم یا بیمارانم» و «مراقبت قلبی یا مراقبت کوید»، طبقه بندی شدند.
نتیجه ­گیری: مراقبت از بیماران کووید 19، برای پرستاران، در بخش مراقبت­ های ویژۀ قلبی، همراه با تجربۀ چالش­ های اخلاقی بزرگی بود؛ اما پرستاران، با تکیه بر تجربه و مهارت خود سهم مهمی در مدیریت چالش ­های اخلاقی و مراقبتی داشتند؛ آن­گونه ­که تعهد حرفه‌ای خود را ارتقا داده و تجربه ­ای ارزشمند را در مراقبت از این بیماران، به­ دست آوردند.

چکیده English

Introduction:
As the primary caregivers of people with COVID-19, nurses were exposed to the ethical challenges and conflicts of caring for their patients. The simultaneous care of cardiovascular patients with COVID-19 has placed a multiplier burden on the nurses of these patients in terms of care and ethics, which causes burnout and psychological deterioration of these patients. Therefore, this qualitative study was conducted with the aim of understanding the ethical challenges of nurses caring for cardiac patients with COVID-19.
Methods & Materials:
A qualitative study was conducted using qualitative content analysis. The participants were nurses in the intensive care unit and respiratory isolation of patients with COVID-19 in Shahid Dr. Chamran Heart Specialized Hospital in Isfahan. Semi-structured and in-depth interviews were conducted with the participants. The data analysis experiment was performed using Gernheim and Landman's qualitative content analysis method and using MAXQDA20.
Results:
The participants in this study included 10 nurses working in the intensive care unit. A total of 204 primary codes were obtained, which were classified into 8 subcategories and 3 main categories, including "legal care or conscientious care", "myself or my patients", and "cardiac care or COVID care".
Conclusion:
Caring for COVID-19 patients for nurses in cardiac intensive care units was accompanied by the experience of enormous ethical challenges. It is suggested that managers and policymakers in this field try to encourage and strengthen a supportive and ethical work environment to better manage intertwined ethical issues and make the right decisions in the context of the pandemic crisis. Healthcare organizations should prioritize the health of nurses during the pandemic and prevent burnout and maintain the quality of patient care by providing psychological, material, and spiritual support.
