چکیده
مقدمه: مولتیپل اسکلروزیس، یک بیماری مزمن التهابی سیستم عصبی مرکزی است. آموزش خودمراقبتی، می­تواند باعث بهبود کیفیت زندگی بیماران مزمن، گردد. پرستاری از راه دور، شیوه­ای جدید، در مراقبت پرستاری محسوب می­گردد. پژوهش حاضر، به عنوان الگویی برای بررسی تأثیر آموزش خود مراقبتی بر رفتارهای ارتقا دهندۀ سلامت، با رویکرد پرستاری از راه دور، در بیماران ام اس، طراحی و ارائه شده است.
روش­ کار: پژهش حاضر، راهنمای اجرای یک کارآزمایی بالینی موازی تصادفی شدۀ دوسوکور، است. حجم نمونه، به کمک نزم افزار G-power، 70 نفر بیمار ام اس، محاسبه و به­طور تصادفی، در دو گروه مداخله(35 نفر) و کنترل(35 نفر) قرار گرفتند. معیارهای ورود به مطالعه، عبارت بودند از: 1. تمایل به شرکت در مطالعه؛ 2. تشخیص قطعی مالتیپیل اسکلروزیس؛ 3. محدودۀ سنی 18 تا 50 سال؛ 4. استفاده از پیام­رسان وات­ساپ؛ 5. حداقل دو سال سابقۀ بیماری؛ 6. توانایی خودمراقبتی؛ 7. دسترسی به تلفن همراه؛ معیارهای خروج، شامل: 1. خارج شدن از مطالعه به هر دلیل؛ 2. ناتوانی در اجرای برنامۀ مراقبت از خود به دلیل بیماری حاد؛ 3. بازگشت بیماری؛ 4. بروز بیماری­های حاد قلبی، کلیوی، تنفسی، گوارشی و بیماری­های متابولیک؛ 5. غیبت بیش از دو جلسه در دورۀ آموزشی.
جلسات آموزشی، سه روز در هفته به مدت شش هفته، برگزار شد. اطلاعات، با استفاده از پرسشنامه­ های دموگرافیک و سبک زندگی ارتقا دهندۀ سلامت واکر، قبل و بلافاصله بعد از مداخله، جمع­آوری گردید. داده ­ها با استفاده از آزمون­های من ویتنی و ویلکاکسون، با استفاده از نرم افزار SPSS نسخۀ 21، مورد تجزیه و تحلیل قرار گرفتند.
بحث: با توجه به شیوع نسبتاً بالای بیماری مالتیپل اسکلروزیس و اهمیت بالای خود مراقبتی در آن به عنوان یک بیماری مزمن و همچنین جدید بودن بحث پرستاری از راه دور در کشور، این پژوهش حاضر می­تواند، شواهد عمیق علمی را بر اساس طراحی کارآزمایی بالینی، در زمینۀ اثربخشی آموزش خود مراقبتی بر رفتارهای ارتقا دهندۀ سلامت، با رویکرد پرستاری از راه دور در بیماران ام اس، ارائه کند.

The effect of self-care training on health-promoting behaviors with telenursing approach in MS patients: study protocol of a Dosokor (double-blind) randomized parallel clinical trial

چکیده English

Introduction:
Multiple sclerosis is a chronic inflammatory disease of the central nervous system. Self-care training can improve the quality of life of chronic patients. Remote nursing is considered a new method in nursing care. The current study was designed and presented as a model for investigating the effect of self-care education on health-promoting behaviors, with a remote nursing approach, in MS patients.
Methods & Materials:
The present study is a guide for the implementation of a double-blind randomized parallel clinical trial. The sample size was calculated with the help of G-power software, and 70 MS patients were randomly divided into two intervention groups (35 people) and control (35 people). The inclusion criteria were: 1. Willingness to participate in the study; 2. Definitive diagnosis of multiple sclerosis; 3. Age range from 18 to 50 years; 4. Using the WhatsApp messenger; 5. At least two years of medical history; 6. Self-care ability; 7. Mobile phone access; Exclusion criteria, including: 1. withdrawal from the study for any reason; 2. Inability to implement self-care plan due to acute illness; 3. Return of the disease; 4. Occurrence of acute heart, kidney, respiratory, digestive and metabolic diseases; 5. Absence of more than two sessions in the training course. Training sessions were held three days a week for six weeks. Information was collected using Walker's demographic and health promoting lifestyle questionnaires, before and immediately after the intervention. Data were analyzed using Mann-Whitney and Wilcoxon tests using SPSS version 21 software.
Discussion:
Considering the relatively high prevalence of multiple sclerosis and the high importance of self-care in it as a chronic disease, as well as the newness of the remote nursing discussion in the country, this current research can provide deep scientific evidence based on the clinical trial design, in the field of effectiveness of self-care education on health-promoting behaviors, with a remote nursing approach in MS patients.
