ارائه مراقبت روان گسسته: یک مانع اخلاقی ارتقای سلامت در بیماران دچار سوختگی

چکیده
مقدمه: یکی از پیامدهای مهم سوختگی بر روی بیماران، تغییر در سلامت روان است. اختلال­های روانی می­تواند، به صورت مستقیم و غیرمستقیم، حیات بیمار را به خطر اندازد. ارائه مراقبت ­های بُعد روان، در کنار سایر ابعاد مراقبتی از نظر اخلاقی، ضروری است. پژوهش حاضر، با هدف آشکارسازی تجربه ­های پرستاران سوختگی از ارائه مراقبت روان، جهت ارتقای سلامت بیماران سوختگی، انجام گردید.
روش­ کار: پژوهش حاضر، از نوع کیفی – تحلیل محتوا( 1402-1401)، در شهر اصفهان، انجام شد. تعداد 9 مشارکت­ کننده، شامل: پرستاران و مدیران پرستاری مرکز آموزشی- درمانی امام موسی کاظم(ع)، وابسته به دانشگاه علوم پزشکی اصفهان، به صورت هدفمند انتخاب شدند. جمع­ آوری داده­ ها با استفاده از مصاحبه ­های عمیق نیمه ­ساختاریافته، انجام گردید. تجزیه و تحلیل داده ­های مطالعۀ کیفی، از نوع تحلیلی- محتوایی، با استفاده از روش گرینهایم و لوندمن، صورت گرفت. در این مطالعه، از چهار معیار لینکلن و گوبا (تأییدپذیری، اعتبار، قابلیت اطمینان و قابلیت انتقال) جهت استحکام داده­ها، استفاده شد.
یافته­ ها: در مجموع، 9 مصاحبه با پرستاران و مدیران پرستاری صورت گرفت. مشارکت ­کنندگان شامل 6 زن و 3 مرد، با  میانگین سنی 24/5 ±  41 سال، بودند. طبقۀ اصلی بر اساس «مراقبت روانی گسسته»، با سه زیر طبقۀ «لزوم ارائه مراقبت روانی»، « نقش پرستاران در ارائه مراقبت روانی» و «موانع مراقبت روانی»، مشخص شد.
نتیجه ­گیری: آماده سازی فردی و سازمانی، جهت ارائه مطلوب مراقبت روان، به صورت بین حرفه ­ای، برای بهبود پیامدهای بیماران سوختگی، به عنوان یک اصل آموزشی - اخلاقی ضروری، می­ باشد.

Providing discrete psychiatric care: an ethical obstacle to improving health in burn patients

چکیده English

Introduction:

One of the significant impacts of burns on patients is the change in mental health. Psychological disorders can directly and indirectly endanger the patient's life. Providing psychological care alongside other dimensions of care in terms of ethical is essential. The present study aimed to explore the experiences of burn nurses in providing psychological care to enhance the health of burn patients.

Methods & Materials:

The current study was conducted qualitatively and descriptively in the years 2022-2023(ten months) in Isfahan. Participants included nurses and nursing managers from the Imam Musa Kazem (AS) Educational Medical Center affiliated with Isfahan University of Medical Sciences, selected purposefully. Data collection was performed using semi-structured in-depth interviews. Data analysis was conducted using the Lundman and Graneheim approach. The study utilized Lincoln and Guba's four criteria (credibility, dependability, confirmability, and transferability)to strengthen the data.

Results:

The nine interviews were conducted with nurses and nursing managers. The participants included 6 women and 3 men with an average age of 41 ± 5.24 years. The main theme identified was "Psychological Care Disruption," which encompassed three subthemes: "The necessity of providing psychological care," "The role of nurses in providing psychological care," and "Barriers to psychological care."

Conclusion:

Individual and organizational preparedness for providing effective psychological care in an interdisciplinary manner is an essential educational and ethical principle for improving outcomes for burn patients.
