بررسی علل عدم مراجعه سالمندان بالای 60 سال شهرستان جهرم به مراکز بهداشتی - درمانی شهری و روستایی ( 1402)

چکیده
مقدمه: در عصر حاضر، یکی از مسائل بهداشتی جوامع، افزایش تعداد و نسبت سالمندان، در قسمت اعظم دنیا است. با توجه به مشکلات و ناتوانی­ های دورۀ سالمندی و لزوم انجام روتین مراقبت ­ها جهت این گروه سنی و از آنجا که تعدادی از سالمندان، جهت دریافت خدمات، به مراکز بهداشتی - درمانی مراجعه نمی­ کنند؛ بنابراین پژوهش حاضر، با هدف بررسی و تعیین سهم هر یک از  علل عدم مراجعۀ سالمندان، به مراکز بهداشتی -  درمانی شهری و روستایی انجام شد.
روش کار: پژوهش حاضر، از نوع توصیفی - مقطعی بود که بر روی 450  سالمند شهرستان جهرم (1402)، انجام شده است. روش نمونه گیری، به صورت  خوشه ­ای چند مرحله ­ای تصادفی و گرد آوری داده ­ها با استفاده از پرسشنامۀ دو قسمتی شامل: سؤالات جمعیت شناختی و عوامل مؤثر بر عدم مراجعه مردان، به مراکز بهداشتی -درمانی اجرا گردید. تجزیه و تحلیل داده­ها توسط نرم افزار آماری spss 21و با استفاده از آزمون­های تی مستقل و آنالیز واریانس انجام شد.
یافته ­ها: براساس نتایج این بررسی، ترجیح دریافت خدمات بخش خصوصی نسبت به بخش دولتی  (8/60±13/54) از مهم­ترین عوامل عدم مراجعه و عامل وجود مسائل شخصی (3/04±5/94) از کم اهمیت ترین عوامل عدم مراجعه به مراکز بهداشتی - درمانی شهری و روستایی بوده است. نتایج این پژوهش، با عواملی مانند: سن (000/=p)، شغل(032/=p)، نوع بیمه  (000/=p)، وضعیت اقتصادی (000/=p) و محل سکونت (000/=p) ارتباط معناداری داشت و با سایر فاکتورها از جمله: جنس، وضعیت تأهل و تحصیلات، ارتباط معنا داری نشان نداد.(05/.< P)
نتیجه­ گیری: با توجه به یافته ­های طرح، به منظور مراجعۀ سالمندان به بخش دولتی، لازم است که عواملی مانند:  بهینه سازی و به­روزسازی تجهیزات پزشکی، فضای فیزیکی مراکز و ارتقای کیفیت ارائه خدمات با تأمین کافی منابع انسانی در حوزه ­های مختلف بهداشتی و دولتی، تلاش کرد تا شاهد افزایش آمار مراجعه­ کنندگان به مراکز باشیم.

چکیده English

Introduction: Today, one of the health issues of societies is the increase in the number and proportion of elderly people in most parts of the world. Due to the problems and disabilities of old age and the need to perform routine care for this age group and because some of the elderly do not go to health centers to receive services. Therefore, the present study was conducted with the aim of investigating and determining the contribution of each of the causes of non-referral of the elderly to urban and rural health centers.
Method: This descriptive-cross-sectional study was conducted on 450 elderly people in Jahrom in 2024. The sampling method was a multi-stage random cluster. Data collection was done using a two-part questionnaire including demographic questions and factors affecting men's non-referral to health centers. Data analysis was done by software. Statistics were done using spss 21 and independent t-tests and analysis of variance.
Results  Based on the results of this survey, the preference of receiving services from the private sector over the public sector (13.54±8.60) is one of the most important factors of non-referral and the presence of personal issues (5.94±3.04) is one of the least important factors for non-referral to urban and rural health centers. The results of this research had a significant relationship with factors such as: age (p=000), occupation (p=032), type of insurance (p=000), economic status (p=000) and place of residence (p=000) and did not show a significant relationship with other factors such as gender, marital status and education. (P<.05)
Conclusion: According to the findings of the project, in order to refer the elderly to the public sector, it is necessary to optimize and update the medical equipment and the physical space of the centers and improve the quality of service provision by providing sufficient human resources in various health and government areas, in order to see an increase in the number of visitors to the centers.
