بومی سازی راهنمای بالینی مراقبت در منزل از بیماران سالمند مبتلا به سکته مغزی:پروتکل مطالعه

چکیده
مقدمه: راهنمای بالینی مراقبت در منزل از بیماران سالمند مبتلابه سکته مغزی، حلقه مفقود شده ای در ارائه خدمات مراقبت در منزل در ایران  است که می تواند نقش مهمی در ارتقا سلامت و بازتوانی بیماران مبتلابه سکته مغزی داشته باشد. این در حالی است که تدوین راهنمای بالینی در ایران، بسیار دشوار خواهد بود و با بومی‌سازی راهنماهای بالینی موجود می توان، از دوباره‌کاری پیشگیری و در  مصرف منابع موجود صرفه جویی نمود. لذا این مطالعه با هدف "بومی سازی راهنمای بالینی جهت مراقبت در منزل از بیماران سالمند مبتلابه سکته مغزی" انجام می‌گردد.
روش کار: پژوهش حاضر، شامل سه گام؛ آمادگی، بومی سازی و نهایی سازی است. مرحله اول (آمادگی) شامل 5 گام (بررسی قابلیت انجام فرآیند بومی‌سازی، تشکیل کمیته اجرایی، انتخاب عنوان راهنما، شناخت و تعیین منابع، امکانات و مهارت‌های لازم و نوشتن برنامه فرآیند بومی‌سازی)، مرحله دوم (بومی‌سازی) شامل 5 گام (تنظیم سؤالات، جستجوی راهنماها و شواهد مرتبط دیگر، غربالگری راهنماهای پیدا شده و محدود کردن آن‌ها، بازنگری ارزیابی‌ها و انتخاب راهنماها و تهیه پیش‌نویس راهنمای بالینی بومی‌شده) و مرحله سوم (نهایی سازی) شامل 2 گام(بازنگری خارجی توسط کاربرهای هدف و تدوین راهنمای نهایی با اجماع نظر متخصصان و استفاده از تکنیک RAM) است.
بحث: جهت بومی سازی راهنماهای بالینی، باید ابتدا با کلید واژه های مناسب، راهنماهای بالینی مشابه را بررسی  و بهترین راهنماها را انتخاب نمودند. سپس، با  استفاده از  نظرات افراد صاحب نظر و ذی نفعان، بر اساس شرایط فرهنگی، اقتصادی، اجتماعی و امکانات موجود، باید راهنماهای بالینی را بازنگری نمودند تا در کشور، قابلیت اجرا داشته باشند.

Localization of clinical guidelines for home care of elderly patients with stroke: A Study Protocol

چکیده English

Introduction:
Clinical guidelines for home care of elderly stroke patients is a missing link in providing home care services in Iran, which can play an important role in improving the health and rehabilitation of stroke patients. This is despite the fact that compiling clinical guidelines in Iran will be very difficult, and by localizing existing clinical guidelines, rework can be prevented and resources can be saved. Therefore, this study is carried out with the aim of "localization of clinical guidelines for home care of elderly patients with stroke".
Method:
The current research includes three steps of preparation, localization and finalization. The first step (preparation) includes 5 steps (checking the ability of carrying out the localization process, forming the executive committee, choosing the title of the guide, identifying and determining the necessary resources, facilities and skills and writing the program localization process), the second step (localization) including 5 steps (choosing  questions, searching for guidelines and other relevant evidence, screening the found guidelines and limiting them, reviewing evaluations and selecting guidelines and preparing a draft of localized clinical guidelines) and the third step ( finalization  guideline) including 2 Steps (external review by target users  and compiling the final guide with the consensus of experts and the use of the RAM technique).
Discussion: 
In order to localize clinical guidelines, first research team should review similar clinical guidelines by appropriate keywords and select the best guidelines. Then, experts and stakeholders should be revised guideline based on cultural, economic, and social conditions and available facilities. So that they can be implemented in the country.
