قابلیت‌ها و چالش‌های توانبخشی از راه دور در بیماران مبتلا به سکتۀ مغزی: یک مرور روایتی

چکیده
مقدمه: توانبخشی از راه دور ارائه خدمات توانبخشی در یک مکان دور با استفاده از فناوری اطلاعات و ارتباطات می‌باشد. به دلیل عدم حضور مستقیم درمانگر و احتمالا کاهش هزینه‌های درمان، این روش می‌تواند یک گزینه مناسب برای توانبخشی طولانی مدت بیماران سکته مغزی باشد. مطالعه حاضر به مروری کوتاه بر قابلیت ها و چالش های توانبخشی از راه دور در بیماران سکته مغزی می‌پردازد.
روش کار : جست‌وجو در پایگاه‌های کاکرین، ساینس دایرکت، پاب مد و گوگل اسکولار در سالهای 2024-1995 انجام شد.کلیدواژه‌ها براساس MeSH شاملStroke ،Cerebrovascular accident ،Telerehabilitation, Telemedicine, Telehealth بودند. در جست و جوی اولیه در پایگاه های اطلاعاتی 20934مطالعه به دست آمد که پس از بررسی عناوین، تعداد مقالات به 625 مطالعه کاهش پیدا کرد. در مرحله ی بعد با بررسی خلاصه مقالات، 8 مطالعه که با اهداف پژوهش همخوانی داشتند انتخاب شده و به طور جامع مورد بررسی قرار گرفتند.
یافته ها: اغلب بیماران توانبخشی از راه دور را ترجیح می‌دهند زیرا علاوه بر کاهش هزینه‌ها فرصت دسترسی به خدمات توانبخشی را برای افرادی که دور از مراکز تخصصی زندگی می‌کنند فراهم می‌کند. از جمله چالش‌های این خدمات چگونگی انجام و تفسیر ارزیابی‌ها از راه دور و فقدان استانداردهای بین المللی در زمینه اخلاق در توانبخشی از راه دور می باشد. به منظور استفاده موثر و صحیح از این روش رعایت اصول اخلاقی، تبیین یک دوره آموزش برای درمانگران و استفاده کنندگان خدمات و چگونگی حفظ امنیت اطلاعات بیماران ضروری به نظر می‌رسد.
نتیجه‌گیری: علاقه فزاینده به توانبخشی از راه دور نشان می‌دهد که این حوزه همچنان به رشد خود ادامه خواهد داد. اما نکته مهم این است که این روش باید به نحوی سازماندهی شود که همان انتظارات توانبخشی حضوری را برآورده نماید. انجام مطالعات بزرگتر و قویتر جهت بررسی کارایی، امکان سنجی، مقرون به صرفه، اخلاقی و عملی بودن توانبخشی از راه دور پیشنهاد می‌گردد.

Benefits and challenges of telerehabilitation in stroke patients: A narrative review

چکیده English

Introduction: Telerehabilitation is the provision of rehabilitation services in a remote location using information and communication technology. Due to the absence of the therapist and the reduction of treatment costs, this method can be a suitable option for long-term rehabilitation of individuals with stroke. The present study briefly reviews the benefits and challenges of telerehabilitation techniques in patients with stroke.
Methods & Materials: The search was conducted in Cochrane, Science Direct, PubMed, and google scholar databases between1995-2024. Keywords included Stroke, Cerebrovascular accident, Telerehabilitation, Telemedicine, and Telehealth. In the initial search in databases, 20934 studies were obtained, and after reviewing the titles, the number of articles was reduced to 625 studies. In the next step, by comprehensively reviewing the abstracts, 8 studies that were consistent with the objectives of the current research were selected and comprehensively studied.
Results: Most patients prefer telerehabilitation because, in addition to reducing costs, it provides the opportunity to access rehabilitation services, especially for people who live far from specialized rehabilitation centers. Among the challenges of these services are how to conduct and interpret remote assessments and the lack of international standards in the field of ethics in remote rehabilitation. To effectively and correctly use this method of observing ethical principles, it seems necessary to explain a training course for therapists and service users and how to maintain the security of patients' information. Conclusion: The growing interest in telerehabilitation indicates that this field will continue to develop. However, the major issue is that this new method should be organized to meet the same expectations as in-person rehabilitation. Larger and stronger studies are suggested to investigate the efficiency, feasibility, cost-effectiveness, ethical, and practicality of telerehabilitation.
