چکیده
مقدمه: تامین منابع برای پاسخگویی به تمامی نیازهای بهداشتی و درمانی در بخش سلامت، حتی برای کشورهای ثروتمند نیز بطور تام رخ نمیدهد؛ لذا مسئله تخصیص منابع و اولویت بندی همواره یکی از چالشهای مدیریتی در نظام سلامت بوده است.
معرفی بیمار: بیمار آقایی 62 ساله به دلیل کاهش سطح هوشیاری توسط اورژانس 115 به بیمارستان منتقل گردید. در آمبولانس، بیمار ایست قلبی میدهد و به دلیل فیبریلاسیون بطنی (VF)، شوک الکتریکی دریافت میکند و احیا میشود. سریعا پس از ورود به بیمارستان در بخش مراقبت های ویژه بستری میگردد. بعد از دو روز هم چنان سطح هوشیاری بیمار پایین بوده و مردمک به نور واکنش کمی نشان میدهد. چند ساعت بعد بیمار دچار تشنج شده و پس از انجام سی تی اسکن برای بیمار، hypoxic brain damage گزارش می شود. بیمار علی رغم اقدامات پزشکی و مراقبت های ویژه در همین وضعیت باقی مانده و در ICU به سر میبرد و هیچ روند بهبودی پیدا نکرده است. اکنون 22 روز از این وضعیت میگذرد. به تازگی یک بیمار جوان 18 ساله در اثر تصادف دچار ضربه مغزی گردیده و به بیمارستان منتقل می شود. این در حالی است که ظرفیت ICU در سایر بخش های بیمارستان وجود ندارد و با کمبود تخت ICU مواجه هستیم.
نتیجهگیری: با تشکیل کمیته اخلاق بیمارستانی و بررسی اندیکاسیونها و مشارکت دادن همراهان بیمار 62 ساله در تصمیمگیری؛ می توان یک تخت در بخش اضافه کرده و مراقبتهایی نظیر ICU ارائه داد تا بیماری که اولویت بیشتری دارد نیز بتواند در ICU نگه داری شود. یک پیشنهاد دیگر نیز آن است که با اضافه نمودن یک تخت اکسترا در ICU هر دو بیمار را در بخش مراقبتهای ویژه نگهداری نمود که در این صورت نیز با توجه به محدودیت منابع، کیفیت خدمات رسانی به همهی بیماران نسبت به درمان استاندارد به یک نسبت کاهش می یابد.

A patient with Hypoxic Brain Damage and a Patient with Stroke: Prioritized in ICU

Introduction: Providing resources to meet all health and medical needs in the health sector is not totally realized even in wealthy countries. Hence, the issue of resource allocation and prioritization has always been one of the management challenges in health systems.
Patient introduction: The patient was a 63-year-old man who taken to the hospital because of a reduced level of consciousness. In the ambulance, the patient had a heart attack and received electric shock due to ventricular fibrillation (VF). Immediately after admission to the hospital, he was hospitalized in the intensive care unit (ICU). After two days, the patient's consciousness was still low and his pupil showed a slight reaction to light. A few hours later, the patient suffered a seizure and the CT Scan results indicated hypoxic brain damage. Despite medical and special care efforts, he has remained in the same situation in the ICU and no improvement has been found. It is 22 days that he has been in the hospital. Recently, an 18-year-old patient has been taken to the hospital due to a stroke in a car accident. However, there is no blank capacity of ICU in other wards of the hospital and we face a shortage of ICU beds.
Conclusion: Through the establishment of the hospital ethics committee, review of indications, and involvement of the 62-year-old patient’s fellows in decision-making, one bed can be added to one of the wards and the necessary ICU services can be provided to the patient with a higher emergency status. Another suggestion is that an extra bed can be added to the ICU in order to keep both patients in this ward. In this case, considering the resource constraints, the quality of serving all patients will similarly reduce from the standard level.
