بررسی سطح دیسترس اخلاقی در پرستاران و پزشکان مراقب از بیماران مبتلا به کووید 19 : یک مطالعه توصیفی مقطعی در سال 1399

چکیده

مقدمه: با توجه به افزایش روزافزون بیماری کووید 19 در جهان و شیوع موج دوم کووید 19 در ایران، روزانه شاهد مرگ بسیاری از هم وطنانمان در بیمارستان‌های کشور هستیم. در این میان کادر درمانی بیمارستان ها، علی الخصوص پزشکان و پرستاران بیشترین درگیری را با بیماران مبتلا به کووید 19 دارند. با توجه به مسئله بسیار مهم حساسیت اخلاقی در این بازه زمانی و اثر آن بر کیفیت عملکرد تیم درمانی، بر آن شدیم تا مطالعه‌ای را با هدف بررسی سطح دیسترس اخلاقی در پرستاران و پزشکان مراقب از بیماران مبتلا به کووید 19 انجام دهیم.
روش کار: این مطالعه توصیفی مقطعی در سال 1399 با نمونه گیری در دسترس بر روی 103 نفر از پرستاران و پزشکان شاغل در بیمارستان پیمانیه شهرستان جهرم انجام شد. ابزار جمع‌آوری اطلاعات در این مطالعه شامل پرسشنامه اطلاعات دموگرافیک و پرسشنامه دیسترس اخلاقی کورلی بود. تجزیه و تحلیل اطلاعات با استفاده از نرم افزار آماری spss نسخه 21 و با استفاده از آزمون‌های آماری توصیفی و استنباطی در سطح معنی‌داری p<0.05 صورت گرفت.
یافته‌ها: در این مطالعه 103 نفر از پرستاران و پزشکان درگیر با بیماران مبتلا به کووید 19 شرکت نمودند. میانگین شدت و تکرار دیسترس اخلاقی، به ترتیب 17.69±47.98 و 16.78±47.82 بوده است. نمره بالاتر نشان‌دهنده شدت و تکرار دیسترس اخلاقی بالاتر است. میزان سطح شدت و تکرار دیسترس اخلاقی در اکثریت کادر درمانی مراقب در حد کم قرار داشت. شدت و تکرار دیسترس اخلاقی در پزشکان بالینی بالاتر از پرستاران و پزشکان عمومی بود اما بین شغل و شدت و تکرار دیسترس اخلاقی ارتباط معنی دار وجود نداشت
(0.05>p-value).
نتیجه گیری: نتایج این مطالعه نشان داد که سطح دیسترس اخلاقی در مطالعه حاضر در سطح بسیار کم قرار دارد. از آنجا که پیامدهای دیسترس اخلاقی، می‌تواند بر کیفیت کار درمانی پرستاران در سیستم سلامت، تاثیرگذار باشد، لذا توجه جدی مدیران جهت به کارگیری برنامه‌های آموزشی در رابطه با مباحث اخلاقی با هدف شناسایی علایم دیسترس اخلاقی و عوامل موثر بر آن و نیز انجام تحقیقات بیش‌تر جهت شناسایی سایر ابعاد دیسترس اخلاقی در دیگر مراکز درمانی، ضروری به نظر می رسد.

Introduction: Due to the increasing number of Covid 19 disease in the world and the prevalence of the second wave
of Covid 19 in Iran, we are witnessing the death of many of our compatriots in the country's hospitals every day. Meanwhile, the medical staff of hospitals, especially doctors and nurses, have the most conflicts with patients with Covid 19. Considering the very important issue of moral sensitivity in this period and its effect on the quality of performance of the treatment team, we decided to conduct a study to investigate the level of moral distress in nurses and physicians caring for patients with Covid 19.
Materials and Methods: This cross-sectional descriptive study was performed in 1399 with available sampling on 103 nurses and physicians working in Peymaniyeh Hospital in Jahrom. Data collection tools in this study included the Demographic Information Questionnaire and the Corelli Moral Distress Questionnaire. Data analysis was performed using SPSS software version 21 and descriptive and inferential statistical tests at a significant level of p<0.05.
Results: In this study, 103 nurses and physicians involved with patients with Covid 19 participated. The mean severity and recurrence of moral distress were 47.98± 17.69 and 47.82± 16.78, respectively. A higher score indicates the severity and recurrence of higher moral distress. The level of severity and recurrence of moral distress was low in the majority of caregivers. The severity and recurrence of moral distress in clinicians was higher than nurses and general practitioners, but there was no significant relationship between job and severity and recurrence of moral distress (p-value <0.05).
Conclusion: The results of this study showed that the level of moral distress in the present study is very low. Since
the consequences of moral distress can affect the quality of nurses' occupational therapy in the health system, so the serious attention of managers to use training programs related to ethical issues with the aim of identifying the symptoms of moral distress and the factors affecting it. It also seems necessary to do more research to identify other dimensions of moral distress in other medical centers.
